Mystery Case: Terson syndrome on CT head.
نویسندگان
چکیده
A 36-year-old woman presented with acute severe headache and generalized tonic-clonic seizures. CT head scan revealed a tentorial acute subdural hemorrhage (ASDH) and posterior globe hyperdensities suggestive of intraocular blood with a right posterior communicating artery aneurysm on cerebral angiography (figure 1). Ophthalmologic assessment confirmed Terson syndrome (figure 2). Terson’s original definition of intravitreal hemorrhage (IVH) complicating subarachnoid hemorrhage has broadened to include ASDH with retinal and preretinal hemorrhages as well as IVH. Plain CT head can demonstrate Terson syndrome, which is easily missed and warrants ophthalmologic input. Significant persisting visual impairment may require vitrectomy with or without laser membranotomy.

PURPOSE Terson syndrome may be overlooked in the acute setting and often requires ophthalmologic intervention to prevent long-term visual loss. In this syndrome, vitreous or retinal hemorrhage results from an abrupt rise in intracranial pressure, leading to retinal venous hypertension and intraocular hemorrhage. Our objective was to determine whether imaging findings could be discovered that mi...

متن کاملTerson Syndrome in Aneurysmal Subarachnoid Hemorrhage: A Case Report
Terson syndrome refers to oculocerebral syndrome of retinal and vitreous hemorrhage associated with spontaneous subarachnoid hemorrhage or all forms of intracranial bleeding. Recent observations have indicated that patients with spontaneous subarachnoid hemorrhage have an 18% to 20% concurrent incidence of retinal and vitreous hemorrhages with about 4% incidence of vitreous hemorrhage alone. Cl...
